On [XXX], at approximately [XXX]., I was involved in a low-speed, front-end collision while making a left turn from [XXX] onto [XXX]. I was following a Toyota Tacoma that stopped suddenly for a pedestrian near the crosswalk. I immediately applied what I believed was maximum emergency braking pressure to my brake pedal. The pedal felt firm, with apparently normal resistance, but I did not experience the deceleration or braking response I expected. Despite my braking effort, my Murano continued forward and struck the rear of the Tacoma. My VIOFO dashcam recorded the incident, including GPS-indicated vehicle speed and time. The displayed speed was approximately 14 mph at impact. I experienced no noticeable strong deceleration before the collision. I did not observe a forward-collision warning or automatic emergency braking intervention. I subsequently had the vehicle examined by a collision repair facility on [XXX] and Tony Nissan in Waipahu on [XXX]. Electronic scans did not identify braking-system fault codes. Tony Nissan performed an approximately 18-mile road test and reported that it could not reproduce the braking concern. Its inspection included checks for hydraulic leaks, brake-fluid level, and brake-booster vacuum leakage. The dealership also found the front brake pads worn to approximately 2 mm. Although the braking problem has not been reproduced or diagnosed, I remain concerned that an intermittent mechanical, hydraulic, or electronic braking malfunction may have occurred during the collision. The vehicle previously received a replacement ABS actuator and electronic control unit under Nissan campaign R1809 in February 2019. I have opened a case with Nissan Consumer Affairs, Case No. [XXX]. Nissan has instructed me not to repair or alter the vehicle pending further contact. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I was stopped in a friend’s driveway in front of her closed garage door to drop her off after an outing. The front sensor had not been alerting me to how close I was to any objects several times the same day. Before my passenger exited, the car suddenly lurched forward and smashed into my friend’s garage door, buckling several panels on the garage door and scraping the paint off the front of my car. I have contacted the dealership to have the car inspected and I’m waiting for an appointment. No police report has been filed. No injuries occurred. There were no warning lamps, messages, or other symptoms (aside from the front sensor not alerting me when I was close to objects in front of me) prior to this incident.

The contact filed the complaint on behalf of the contact's son. The contact's son was the operator of a 2026 FTL MM Business Class M2. The contact stated that while the contact's son was driving at an undisclosed speed, the vehicle was uncontrollable. The vehicle independently drove off the road, crashed into a covert, crashed into a fence and then crashed into a hay bail. In addition, the vehicle rolled three times before coming to a stop. The airbags failed to deploy as intended. The vehicle was towed to a local tow yard. The contact's son was DOA. A Police report was filed. The State Trooper informed the contact that there was no indication of why the vehicle left the road. The contact researched and discovered a recall, NHTSA Campaign Number: 25V760000 (Suspension), which had a similar failure description. The manufacturer was contacted and made aware of the failure. The contact was advised to contact the local freight liner dealer.

My 2022 Mercedes-Benz EQS 450+ repeatedly showed "Active Brake Assist Functions Limited," "Active Emergency Stop Assist Inoperative," and Blind Spot Assist warnings, photographed by September 23, 2026. On September 24, I reported these issues and showed photos to an authorized dealer. Electronic repairs were performed, but warnings returned. The repair record lists Blind Spot Assist as "not verified"; I have no record of an Active Brake Assist test. On October 8, while turning left across oncoming traffic, the car was in a collision. Automatic emergency braking did not activate. I know the distinctive sudden braking sensation from previous experience. I cannot determine whether activation was expected in these circumstances. Police attended, and the insurer declared a total loss. The car is at Copart; preservation for inspection has been requested but not confirmed. Please investigate a possible safety-system defect.

On [XXX], at approximately [XXX] in Baker, California, my 2022 Tesla Model 3 contacted charging equipment and protective barriers while moving slowly into a charging space during what I observed to be an Autopark maneuver. I was watching the vehicle camera display and was not manually steering or accelerating. The car did not show or sound any warning before impact. It was not raining. No one was injured, there was no fire, and no police report was filed. There was visible damage to the side of the vehicle. No one has inspected the damage yet. The vehicle will be available for inspection after my trip. I reported the incident to Tesla and requested preservation and review of available records. The data export I received did not contain incident-specific camera recordings, Autopark/control logs or EDR data that I could identify. No recording USB drive was located. I have damage photographs and a charging-session record. The cause has not been independently determined, and I have not received a substantive engineering finding. I am reporting a possible safety issue with Autopark for investigation. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The vehicle was in a significant crash which smashed in the front end and popped both front tires during which the airbags did not deploy. The vehicle is available until the insurance company moves it to salvage. Since the airbag did not deploy my fiance suffered a concussion from hitting the steering wheel. The problem has not been investigated by anyone other than insurance. No warnings of system problems were on prior to the failure to our knowledge.

the brake hubs started smoking and got hot. 9/13/26 I was rear ended but no warning was initiated or airbag. now I have no brake pads and was unable to stop. I had an oil change due 30thousand miles but Affenburg in Shilo IL turned me away. I'm not happy with the maintenance people for not helping this last year. I was not informed of my last recall or brake extension. this is a new car 2022 I expect better safety reminders and connection from Volkswagen. The bad brake system doesn't work. I do love my Taos as I feel it was made for me but the company is bad about honoring warranty issues.
